Introduction to Central Government Health Scheme (CGHS)
The **Central Government Health Scheme (CGHS)** is a comprehensive, subsidized healthcare program established by the Ministry of Health and Family Welfare, Government of India. Initiated in 1954, it serves as the healthcare backbone for Indian Central Government employees, pensioners, members of parliament, judicial members, freedom fighters, and their eligible dependents.
CGHS provides holistic medical care under multiple health systems, including Allopathic, Homeopathic, Ayurvedic, Siddha, Unani, and Yoga. With a massive network of wellness centres, polyclinics, and tie-ups with top-tier private empanelled hospitals, the scheme ensures that government personnel have access to premium outpatient treatments (OPD), inpatient care (IPD), diagnostics, and free medicine distribution.
CGHS Subscription Rates: 7th CPC Slabs & Rules
The monthly premium contribution for CGHS is automatically deducted from a serving employee's monthly basic salary. For pensioners, it is paid annually or through a one-time lifetime fee. The subscription rates are defined in flat slabs based on the employee's corresponding **7th Central Pay Commission (CPC) Pay Level**:
| 7th CPC Pay Matrix Levels | Monthly Subscription (₹) | Annual Cost (₹) | Eligible Employees / Pensioners Slabs |
|---|---|---|---|
| Level 1 to 5 | ₹250 | ₹3,000 | Multi-Tasking Staff (MTS), LDC, Constables, etc. |
| Level 6 | ₹450 | ₹5,400 | Assistants, Sub-Inspectors, PRT Primary Teachers, Station Masters |
| Level 7 to 11 | ₹650 | ₹7,800 | Section Officers, Inspectors, Lieutenants, TGT & PGT Teachers |
| Level 12 and above | ₹1,000 | ₹12,000 | Under Secretaries, Directors, Joint Secretaries, military Colonels+ |
Note: The subscription rates were last updated under the 7th CPC recommendations and are standard across all states, autonomous bodies, and ministries in India.
Private Empanelled Hospital Ward Entitlement (Revised Slabs)
When a CGHS beneficiary is referred to a private empanelled hospital for inpatient department (IPD) treatment, their **ward category eligibility** (General Ward, Semi-Private Ward, or Private Ward) is strictly determined by their monthly **Basic Pay**.
The Ministry of Health and Family Welfare revised these basic pay slabs in **October 2022** to reflect the updated pay scale standards. The current ward entitlements are as follows:
1. General Ward Entitlement
Available for employees and pensioners with a monthly basic pay **up to ₹36,500**. This covers standard shared ward accommodations without extra charges.
2. Semi-Private Ward Entitlement
Available for employees with basic pay ranging from **₹36,501 to ₹50,500**. Covers dual-sharing air-conditioned rooms.
3. Private Ward Entitlement
Available for employees with basic pay **above ₹50,500**. Covers single occupancy air-conditioned private rooms.
*Disclaimer: If a beneficiary requests a ward higher than their entitlement, they must pay the difference in room rent directly to the empanelled hospital, except in emergency cases certified by the medical superintendent.*
Pensioners CGHS Card Benefits: Annual vs Lifetime Subscription
Retired Central Government employees have the option to continue their CGHS coverage post-retirement. CGHS offers two flexible contribution models for pensioners:
Annual Renewal Card
Pensioners pay the contribution equivalent to 12 months (determined by their last drawn pay level) on a year-by-year basis. The card requires annual renewal by depositing the contribution fee.
One-Time Lifetime Card
Pensioners can make a **one-time contribution payment equal to 10 years of subscriptions** (e.g. 120 months contribution). This guarantees lifetime card validity with zero future renewal contributions. If a pensioner passes away, the surviving spouse or eligible family pensioner can continue using the card with no additional fees.
CGHS vs Fixed Medical Allowance (FMA)
Central Government pensioners residing in non-CGHS covered areas (outside CGHS cities) can opt out of the CGHS OPD facility and instead choose a **Fixed Medical Allowance (FMA)**. FMA is currently paid at a flat rate of **₹1,000 per month** alongside the monthly pension.
Pensioners should evaluate this carefully:
- Fixed Medical Allowance (FMA): Best for those living in remote rural areas with no CGHS Wellness Centres nearby who only have minor outpatient medical expenses. FMA does not cover inpatient hospitalization bills.
- CGHS Membership: Ideal for pensioners living in cities with empanelled hospitals, or who anticipate higher hospitalization and diagnostic costs, since CGHS covers major cashless surgeries and specialist treatments.
8th Pay Commission Projections for CGHS
With the 8th Central Pay Commission (CPC) expected to recommend a salary restructure in 2026, both basic pay levels and contribution rates are anticipated to adjust. If a fitment factor multiplier (such as 2.57x or 2.86x) is applied, basic salaries will rise significantly.
Historically, CGHS premiums are scaled proportionally during pay commission transitions to match the higher budget pools. Our calculator allows users to preview projected rates:
- Projected Slabs: Monthly contributions could scale from ₹250 to ₹650 (for lower levels) and from ₹1,000 to ₹2,600 (for officers) to match the new basic pay ranges.
- Projected Ward Thresholds: The basic pay limits for Private and Semi-Private ward categories are expected to be multiplied by the fitment factor (e.g. Private ward threshold scaling from ₹50,500 to ~₹1,30,000), keeping entitlements balanced.
